"CPO", "prosthetist", "professional" are defined interchangeably as any qualified prosthetist (Certified Prosthetist/Orthotist (CPO) and/or other professional with the same required credentials under a differing name, potentially including but not limited to: "prosthetist/orthotist", "CPO", "prosthetist/bandagist", "orthopaedist", "orthopaedic technician"), according to the legal, regulatory, and/or professional requirements in the Customer's jurisdiction and/or in which each order for a device is placed (as applicable), and are able to legally prescribe devices to user within the applicable jurisdiction, are judged by the customer to be of sound mind, professional capacity and competence, and who have appropriate professional indemnity insurance. It is the responsibility of the customer to ensure that any individual who is described as a CPO as defined in the General terms under which devices as supplied, does indeed meet the definition of CPO provided and has the legal ability to prescribe devices as specified.
Company must be a registered legal entity and have correct VAT status, e.g. registered or under the registration limit (exceptions may apply).
